Your AFA Safety Committee members can provide paper copies of incident reporting forms,

or download them yourself:

one for Aircraft Air Quality & Pesticide incidents (pdf or Word), and

one for All Other incidents (pdf or Word).

Carry these forms with you, and share them with your flying partners.

Use these AFA forms to document safety, health and security incidents as they occur.

Submit completed reports to your local Safety Committee.

 

NOTE: These AFA forms are in addition to those provided by your company;

reporting to your company using their form(s) should be your first priority.
